Planning My Way, a project of the Cunniff-Dixon Foundation, is a comprehensive, no cost guide, that helps you and your loved ones approach difficult conversations and make informed health decisions.
Only one person is truly qualified to tell your health care providers and your loved ones how you feel about end-of-life care issues and that's you.
Our Mission
The Cunniff-Dixon Foundation (CDF) is focused on enriching the patient-provider relationship near the end of life through education, collaboration and recognition. Through Planning My Way, we serve the individual and their caregivers by guiding them through a comprehensive end-of-life care decision plan.
